Remove storage of apps/FOUserAgent from fo/FONode in favor of storing it once in a higher level class (currently Driver), and accessing it from there. Cleaner design, uses less memory, at the expense of some additional processing time to come up the tree to find the information.
Remove all storage of fo/FOInputHandler from within the FO Tree itself, in favor of storing it one time in apps/Document and getting it from there when needed. Makes everything cleaner, and uses less memory, at the probable expense of some processing time (to go up the tree to get to Document).
1. Remove fo/FOTreeExternal and layoutmgr/LMFOTreeExternal (in favor of Visitor concept, see item 2)
2. Add fo/FOTreeVisitor and layoutmgr/AddLMVisitor
3. Remove all occurrences of addLayoutManager in fo/FObj subclasses, replacing them with equivalent methods in AddLMVisitor

Add BreakPossibility style LayoutManager code as an alternative to
Keiron's "direct area creation" method. Not currently enabled: to do
so, one must make 2 changes in the source.

initial implementation of line area layout processing
all inline area and their managers are managed by the
LineLayoutManager. This class fills a line with inline
areas and then does the alignment on all the inline areas.
elements which create inline areas add their managers to the
list that is used by the line layout. These managers have
a retrievable index of inline areas.

the element makers are associated with the element mapping
attributes are passed to the element object the object can then
use the attributes to make the properties
added default value for element handling, makes it easier to
ensure all elements of an external xml markup are loaded (eg. svg)

Unfortunately breaks marker support.
(AreaTree getNextPage and getPreviousPage return the current page)
XSL-FO with markers is not a good idea until it is fixed.

Partial implementation of keep-with-next (doesn't yet remove area from previous page when fo is moved to next page to ensure keep). The layout method on each FO now returns a Status object rather than an int to allow for more information to be passed back in the future.
