FilledArea now properly applies the variation factor, when a page
number citation is resolved. It (and each InlineArea) may have an
adjustingInfo object for that purpose. Adapted leader_toc testcase to
the new, correct, results; removed page-number-citation_forward-ref
testcase because it duplicates the leader_toc testcase.

The text is split into WordAreas and SpaceAreas: the latters (when the font is multibyte) are not affected by the Tw operator, so they are shifted in the inline progression dimension instead.
The behaviour of the other renderers should be the same as before this change.

For area.Blocks created in BlockLayoutManager.getParentArea(), I set the
width of the Block to that of its parent block.
This fixes a layout problem where
<fo:table-cell><fo:block border-bottom="1pt solid black">some text</ ></ >
was causing a line to draw across the entire page, because the cell width present in
fo:table-cell's Block was not copied into the inner fo:block's Block. (This problem did not
occur if the border-bottom attribute was set in fo:table-cell.)
