Use object id provided by working tree iterator for submodules
The working tree iterator now supports providing an object id
for submodule entries and this value should be used instead
of recomputing it again in AddCommand.

Keep submodules in index that are missing in working directory
Submodules present in the index but missing from the working
directory should not be staged for deletion when AddCommand
is called with the update flag set to true.
This mirrors the behavior of CGit. Submodules can still be
staged for deletion by running by using the RmCommand.

Retain executable mode of existing files on Windows
Currently files in a repository marked as executable will have
that mode unset when modified and committed on systems that
do not support detection of this mode since the working tree
iterator will never report this mode for any entries.
This change updates WorkingTreeIterator to be able
to determine the target file mode to be used for the index
through consideration of the configured WorkingTreeOptions.

Its confusing that a new TreeWalk() needs to have reset() invoked
on it before addTree(). This is a historical accident caused by
how TreeWalk was abused within ObjectWalk.
Drop the initial empty tree from the TreeWalk and thus remove a
number of pointless reset() operations from unit tests and some of
the internal JGit code.
Existing application code which is still calling reset() will simply
be incurring a few unnecessary field assignments, but they should
consider cleaning up their code in the future.

When the assume unchanged flag is set the Add command must not update
the index for this file if any changes are present in the working
directory.

This makes it easier for abstract tools like AddCommand to open the
file from the working tree, without knowing internal details about
how the tree is managed.

Allow client of Add command to set a WorkingTreeIterator
This is e.g. useful when a client of the AddCommand has
additional rules to ignore files. In Eclipse a resource can
be set to derived or be excluded by preferences.

Move ignore node handling into WorkingTreeIterator
The working tree iterator has perfect knowledge of the path structure
as well as immediate information about whether or not an ignore file
even exists at this level. We can exploit that to simplify the
logic and running time for testing ignored file status by pushing
all of the checks down into the iterator itself.

The new Add command adds files to the Git Index.
It uses the DirCache to access the git index. It
works also in case of an existing conflict.
Fileglobs (e.g. *.c) are not yet supported.
The new Add command does add ignored files because
there is no gitignore support in jgit yet.
